I know you said you are looking for a hand-cranked one, so this might not help. But...I have the attachment for the Kitchenaid mixer and it works great, if you have a mixer.

if you are talking about the old fashioned cast metal ones, they still sell them new in many places, i have picked them up at flea markets and yard sales, just make sure they are not rusty...

I have my mom's old one. However, whatever it was painted with comes off on the food. Since I'm not sure what is in the paint or the grinder, I just keep it as a memento. That's why I'm looking for a new one. I saw a really nice looking one at Lehmans.com, but don't want to spring for it if it's not a good brand.
