Check out this site as it has instructions for grading a sewing pattern to different sizes as per industry standards.

There is a great book that I have in my collection titled:
"Designing Apparel through the flat pattern" Mine is the 6th edition and the publisher I think is Fairchild fashion & merchandising group, New York.
